Impact: API cleanup
All initialized requests must and are guaranteed to have rq->q set.
Taking both @q and @rq parameters doesn't improve anything. It only
adds pathological corner case where @q != !@rq->q. Kill the
superflous @q parameter from blk_rq_map_{user|kern}*().
