[PATCH 09/11] x86, smap: Turn on Supervisor Mode Access Prevention

If Supervisor Mode Access Prevention is available and not disabled by
the user, turn it on.  Also fix the expansion of SMEP (Supervisor Mode
Execution Prevention.)
