Hunk #1 succeeded at 17 with fuzz 2 (offset 2 lines).
I think it's because the line:
18: #include <linux/kernel.h>
is already present, and patch tries to add it again, so the result is:
18: #include <linux/kernel.h>
19:
20: #include <linux/kernel.h> // for panic()
